Intake Forms Guide

Since our office is still currently operating through telemental health, we have prepared a guide to help with sending in your intake forms. If you have access to a computer and scanner, its as simple as printing off the documents and scanning them back in once filled out. However, if you do not have a scanner, we suggest downloading Adobe Acrobat from your respective smart phone’s app store and following the directions below.

If you do not have a printer or scanner, please follow the guide below:

We suggest downloading Adobe Acrobat or similar application from your app store on your smartphone. Download the intake paperwork from our website under forms. Open the document and fill out each field and sign the signature blocks.

Then save the document as a PDF and email them to info@fairwindsmentalhealth.com.

If you have a printer but do not have a scanner please follow the guide below:

We recommend for the easiest and simplest method is to fill out the forms from your computer, print them, and then to scan them using your smartphone. You can also just print them and fill them out by hand as well.

We are here for our First Responders

It is our pleasure to offer a new program to provide aid during this worldwide crisis. We have initiated a same-day or next-day appointment availability for our overworked and highly stressed caretakers that are fighting this horrific battle against Covid-19. If you or your loved one works in emergency care, doctors offices, EMT, Fire or Police we are here to help. We can offer appointments over the phone or via video conference for your convenience. No specialty applications or technology is needed. Give our office a call and schedule when you have a moment. It is our honor to provide our community with care and support through this time. We wish to thank all those who putting their lives at risk that are fighting on the front line.

COVID-19 Office Safety Information

During these uncertain times, we are focused on minimizing the risk to our patients and the community. Due to the nature of this virus and the need for containment, our office has currently ceased all face to face appointments during this time. We do not have an immediate date at which we will return to normal practices. We will keep all patients informed of the status of our office practices and when visits will return to normal. In the meantime we will be providing telephonic therapy or secure video conference based therapy. We ask that all patients fill out the new Informed Consent for TeleMental Health Services and either fax to 817-735-4688 or email to our office front desk at info@fairwindsmentalhealth.com. Please note that we cannot guarantee end to end security through email. We understand that at this time things might be chaotic, but it is with honor that we continue to offer care to our established patients and be a beacon of light for new comers. We pray for the safety and wellness of everyone during this difficult time.
